Objective: It is currently available markers for the early detection of diabetic nephropathy (DN), the leading cause of end stage renal disease, has several limitations. There is ample evidence from previous studies on the role of several circulating microRNAs (miRNAs) in the early development of DN. This study aims to describe the expression of miRNA-377, miRNA-93, miRNA-25, miRNA-216A, and miRNA-21 in a sample of type 1 diabetic children and adolescents to explore their relationship with DN and several indices of renal injury. Patients and methods: Seventy type 1 diabetic patients with diabetes duration of 5 years or more, recruited from Children’s Hospital, Faculty of Medicine, Cairo University. Quantitative real-time reverse-transcription PCR (qRT-PCR) was used to measure the expression of the above mentioned miRNAs in serum and to assess its relationship with the DN, and the risk factors studied.
Results: There were significantly higher percentage of up-regulation of miRNA-377 and miRNA-93 (P = 0.03, 0.02, respectively) in addition to significant down-regulation of miRNA-25 (P = 0.01) in patients with DN than in patients without DN. In patients with DN, the expression of miR-216A were significantly negatively correlated with creatinine (r = -0.4, P = 0.04) and positively correlated with eGFR using creatinine (r = 0.5, P = 0.03). In the same group, the expression of miR-21 was positively correlated with urinary cystatin C (r = 0.6, P = 0.01) and negatively correlated with e-GFR using cystatin C (r = -0.6, P = 0, 01).
MiRNA-93 was associated with an increased risk (odds ratio = 15, 95% CI = 12.03 to 24.63, P = 0.01), whereas miRNA-25 was associated with a decreased risk of albuminuria (odds ratio = 0.15, 95 % CI = 0.08- 0.55; P = 0.03)
Conclusion: miRNA-377, miRNA-93, miRNA-216A, and miRNA-21 may be involved in the pathogenesis of DN, whereas miRNA-25 may have a reno-protective role. More studies are needed to document the value of these miRNAs as diagnostic biomarkers and therapeutic targets in DN.

Extensive physiological and pathophysiological differences exist in paired organ systems. However, the molecular basis for this difference is unknown. We previously reported that miRNAs are differentially expressed (DEMs) on the left and right kidneys of normal mice.
Here, we identified Dems on the left and right eyes, lungs, and testes of normal mice via RNA sequencing. As a result, we identified 26 Dems in the eye, with 23 higher and 3 lower in the left eye compared to the right eye; 21 Dems in the lungs, with 15 higher and lower 6 in left lung compared to right lung; and 54 Dems in the testicles, with 6 higher and 48 lower in the left testicle than the right testis. ten microRNAs (miRNAs) are subsequently examined by quantitative PCR test, and seven of them were confirmed. In addition, correlation analysis was conducted between paired organ miRNA expression and miRNA expression of a variety of body fluids.
